Check Yourself Questionnaire

Students at participating schools take a brief, interactive and secure web-based questionnaire called Check Yourself that provides instant personalized feedback about health behaviors and helps to determine whether a student may need additional support. Students answer questions about their strengths, goals, coping strategies, substance use, mental health, and safety.

Please note that all questions are not asked to all students. Questions vary by grade level and branch according to previous responses as noted in red.

What Happens Next?

Based on Check Yourself results, an SBIRT team member will connect with students who requested support or indicated a potential need. Students may have short, ongoing conversations with a Prevention & Intervention Specialist or school staff member that focus on:

  • Student strengths and abilities
  • Creating a shared goal with the student and their family
  • Connecting the student to resources and strategies for success

Parents/guardians will always be contacted if students report a safety concern.
